Purpose and limitations

Pain Map Tool organizes symptoms you enter into a report for discussion with your healthcare provider. It does not diagnose, assess urgency, recommend treatment or replace professional care. Do not use the tool to decide whether to delay care.

Your entries and reports

Enter only information you are entitled to use. You may save, print and share your own reports. Entries are not a permanent medical record: refreshing or closing the page can erase them. Keep downloaded files secure and check report content before sharing.

Translation

English translations are automated and may contain mistakes. Keep the original wording and ask your healthcare provider or a qualified interpreter to check meaning. A translated report is not a certified medical translation.

Responsible use

Do not submit malicious code, attempt unauthorized access or overload the website or translation service. Features may be unavailable during maintenance or service interruptions.
